Please register as soon as possible, because if we encounter capacity issues, those who registered later may have to attend remotely.

Reservations are required in order to comply with the Covid-19 guidelines that we have established for the safety and well-being of our congregants.  All attendees must be double vaccinated, boosted, and masked.

If you have not yet done so, send us the vaccination certification for each High Holiday attendee by September 20.  Take a picture or scan of their vaccine card, and either mail it to us with this form, or attach it to an email addressed to ahavassholomnewarkservices@gmail.com
